Far from the reality
Never fight a knife with bare hands if you have a chance to run away.
If dude feels pain and is giving up the weapon it works.
At the point dude has knife on your neck that is a static hold… you pretty much have to fight back. This isn't a knife fight this is a hostage situation.
They probably are not expecting you to fight back and will be lacking. People tend to get over confident or careless. In the moment of lack of focus you can use element of surprise to attack and defeat him.
If he was actually attacking you would already be cut. That isn't the situation so this defense seems realistic to me.
