Knife attacks happen so quickly that it’s important to have trained prior to an attack, rather than trying to figure out a self defense plan in the heat of the action.
Knife attacks can also happen from all different angles including from the top. Therefore, it’s important to learn the proper defense against a knife attack coming from the top.
This can really be a frightening moment, when an assailant grabs your shirt with one hand and starts stabbing you from the other knife hand coming from the top.
Having yourself trapped in that way and having knife stabs coming to the upper body and face is an extremely dangerous situation.
In that case, it’s important to react quickly and stop this attack rapidly.
First off, you must get rid of the hand grabbing your shirt by pushing it away as you’re moving in forward in an angle. You can try to establish “chest to back” with your opponent. That would place you in a favorable position.
If you’re at a fair distance from your attacker, you can start kicking him with front kicks to keep him at bay.
You need to strike your attacker as quickly as possible and jam that knife. You need to change the “predator/prey” mentality as fast as you can, to stay safe in such a brutal attack.

Can you do a video on if you are pinned down with the knife to your throat. It seems like a screwed position to be in and I didn't know what to do when it happened to me but I don't want to feel powerless like that again

Try and mention that the best defense against a knife is running away. There is no reason to fight an armed attacker if you don't have to
For a video you should analyze fights and tell us what they did right and what they did wrong and what they should have done I know you've done it before but it would be cool if you do it again
My take away:
1. Be situationaly aware.
2. Keep your distance
3. Defeat the leveraging arm.
4. Keep your own force multiplier on you.
rule nr1 if you don't have a weapon is to always try and run away or get a physical barrier between you and the knife attacker. barehand against a knife is when you are trapped and there are no other options. like you don't even have ur own belt to swing at the attackers face or hands..
Hey there Nick; a great video, as always. If I may: One seemingly crazy but highly effective strategy that I've found to work well in cases like this is to startle the attacker by spitting (the bigger and juicer the better) on his face as you're going in to close the distance and trap the knife away from him etc. This is a highly distracting [and to most people an extremely gross] move that the attacker is most likely not expecting to come from an individual who's under attack, hence it buys you a precious second or two to launch your counterattack, while his brain is trying to figure out what's going on and how to deal with this highly unorthodox, gross, and unexpected situation. A jacket, a chair, an object may not always be available to assist one against such an attacker, but the spit is always yours, if you keep your wits about you and don't dry up the saliva, so-to-speak.
